Berberine and glucose metabolism

Multiple meta-analyses of randomized trials show a metabolic signal, but most trials were small, short, and of generally low methodological quality.

The question
What do controlled human studies and meta-analyses show about berberine and markers of glucose and lipid metabolism?
Study design
Two meta-analyses of randomized trials (Dong 2012: 14 trials, 1,068 participants; Lan 2015: 27 trials, 2,569 patients), a cell and rodent AMPK study (Lee 2006), a small clinical trial in adults with elevated glucose (Yin 2008), and a human CYP inhibition study (Guo 2012). [1][2][3][4][5][6]
What they found
Across the pooled trials, berberine plus lifestyle change lowered fasting glucose and improved lipid markers more than lifestyle alone, with no clear advantage over standard oral glucose-lowering drugs in Lan 2015. In lab models, berberine activated AMPK and increased glucose transporter movement in muscle cells. In healthy men, two weeks at 300 mg three times a day inhibited CYP2D6, CYP2C9 and CYP3A4, so it can change blood levels of many medicines. [1][2][3][4][5][6]
Limitations
Most trials were small, short, and conducted in China, with generally low methodological quality. Surrogate metabolic markers are not the same as hard clinical outcomes. Drug-interaction risk is real.
What this does not mean
Berberine is not a substitute for prescribed care. It is a candidate SKU, not for sale. If we offer it, the only claim will be structure/function: supports healthy glucose metabolism. NIH's NCCIH notes that goldenseal (a berberine-containing herb) may affect glucose control in people taking metformin, and that people who are pregnant or breastfeeding should not use it.
